Each July, the online Back-to-School Forms become available for the upcoming school year through your PowerSchool Parent Portal account. Online submission of these forms is required before your student(s) may attend school in the fall.
The 17/18 Back to School form is now available through the Parent Portal account.
If you have not yet created your Parent Portal account or have forgotten your login information, contact your student's school. When school is not in session, contact Maumee High School at 419-893-8778 or Gateway Middle School at 419-893-3386. Secretaries at these buildings have the information for all families in the district.
